Chicago Med returned for its 12th season on Wednesday, October 7, 2026, with a premiere that put Gaffney Chicago Medical Center under extreme pressure. A three-month time jump placed the hospital in a very different state than where Season 11 ended, with Sharon Goodwin (S. Epatha Merkerson) away from her post and a new administrator running things.

Dr. John Frost (Darren Barnet) treated an 8-year-old patient named Dakota who had lost her sight. Doctors found nothing wrong with her eyes and determined her blindness was a trauma response. A musician named Bruno had brought her in and encouraged her throughout her treatment.
The episode took a dark turn when a car crashed through the emergency department doors. The driver was Henry, the same man who had opened fire at the wedding. Bruno was killed in the crash. Ripley was left to tell Frost what had happened. A mix-up was later cleared when Dakota’s mother was found to be alive.
Sharon Goodwin’s Battle to Save Her Career
The premiere picked up three months after the Season 11 finale, where board member Miranda Lewis (Orlagh Cassidy) accused Goodwin of leaking Dr. Theo Rabari’s private medical files and asked her to resign.
Goodwin took a voluntary leave from Gaffney, and Mallory Sloan (Rebecca Breeds) stepped in as executive director. The episode ended with a major revelation. Nurse Doris (Lorena Diaz) arrived at Goodwin’s home and confessed.

Doris admitted she had leaked Rabari’s medical file. Showrunner Allen MacDonald explained in an interview with TVLine that the writers settled on Doris because it subverts expectations. He noted that Doris carries guilt that grows as the hospital struggles without Goodwin, and the moment of silence for the shooting victims became her turning point.
“She’s also in this war basically, with Miranda, and Miranda’s holding all the cards, and Miranda’s winning,” MacDonald told Us Weekly about Goodwin’s situation. “So this is the beginning of a battle.”
Hannah and Archer: Parents First, Everything Else Unclear
Hannah Asher returned to the hospital with baby Mabel to visit Archer and brought food for him. She explained her co-parenting arrangement to colleagues and confirmed she would be on maternity leave for a few weeks before returning to work.
The relationship between Hannah and Archer remains undefined. Archer confessed his feelings for her in the Season 11 finale, but the two have not officially become a couple. MacDonald told TV Insider that Hannah and Dean avoid defining their romantic connection while caring for their newborn daughter.
“The theme for the season is just being uncomfortable with the unknown,” MacDonald told PEOPLE.
Where Are Lenox and Ripley
Dr. Caitlin Lenox (Sarah Ramos) was absent from the premiere. The episode revealed she is in Zurich participating in a prion drug trial. Others at the hospital believe she is on a three-month sabbatical to visit family. She and Ripley are making their long-distance relationship work through video chats. Lenox received a terminal prion disease diagnosis in Season 11, and the trial represents her best chance at treatment.
