Kim Ji Won is back on television, and her return is already breaking records. The SBS medical noir drama Doctor X: Mafia in White premiered on October 9, 2026, and it pulled in the highest first-episode viewership of any miniseries that has aired in 2026.
According to Nielsen Korea, the debut episode scored an average nationwide rating of 11.4 percent, with a peak of 12.9 percent and a Seoul metropolitan area rating of 11.2 percent. No other miniseries debut this year has reached those numbers.
What Made the Premiere So Big
The first episode of Doctor X did more than just hit double digits. It became the most-watched show of any kind to air on a Friday in Korea, and it also ranked first among all programs that aired during the week in the 2049 demographic, with an average of 3.7 percent and a peak of 4.3 percent.
For comparison, the premiere of Agent Kim Reactivated, which went on to become one of SBS‘s biggest hits of the year, opened at 9.5 percent. Doctor X beat that by nearly two percentage points right out of the gate.
The second episode, which aired on October 10, climbed even higher to 14.9 percent nationwide, with a peak of 17.4 percent. That kind of jump between episode one and episode two is rare and shows that viewers who tuned in for the premiere came back the very next night.
A Story About a Genius Surgeon Fighting a Corrupt System
Doctor X: Mafia in White follows Gye Soo Jung, played by Kim Ji Won, a brilliant surgeon with unmatched skills who arrives at Guseo University Hospital through a doctor staffing agency. She quickly finds herself at odds with the hospital’s powerful figures, who care more about money and influence than patients.
The drama is a Korean adaptation of the hit Japanese series Doctor-X: Surgeon Michiko Daimon, which starred Ryoko Yonekura and ran for seven seasons from 2012 to 2021 before getting a feature film in 2024.
Kim Ji Won‘s character is cold, stubborn, and willing to break rules when lives are on the line. She challenges senior doctors openly and performs surgeries that others refuse to touch. The show mixes tense operating room scenes with power struggles behind hospital doors.
This is Kim Ji Won‘s first television role since Queen of Tears ended in 2024. That drama started with a 5.9 percent rating and finished at 24.9 percent, making it the highest-rated drama in tvN history.
Who Else Stars in Doctor X
Joining Kim Ji Won is Lee Jung Eun as Jang Hee Sook, the head of the medical staffing agency that places Gye Soo Jung at the hospital. Lee Jung Eun is known for her roles in Parasite and My Liberation Notes. She also worked with Kim Ji Won in My Liberation Notes.
Son Hyun Joo plays Boo Seung Kwon, a senior figure at the hospital with his own agenda. Kim Woo Seok rounds out the main cast as Park Tae Kyung.
The drama is directed by Lee Jung Rim, who previously worked on VIP and The Devil Judge, and written by Pyeon Seong Geun.

Viewers and Critics Are Split on Medical Accuracy
Strong ratings have not stopped some viewers from pointing out problems with the show’s medical scenes. Criticism online has focused on things like a surgeon making unilateral decisions during a multi-specialty operation and a scene where surgery is performed after a hernia had already been treated.
Namgung In, an emergency medicine physician at Ewha Womans University Mokdong Hospital who worked as a medical consultant on the series, addressed the criticism on social media.
“If you see something medically inaccurate, you can assume that the consultant and the production team disagreed, and the consultant lost the argument.”
Some viewers defended the show, saying that fictional medical dramas always take creative liberties. Others praised the fast pacing and Kim Ji Won‘s performance as the standout elements of the premiere.
Kim Ji Won’s Star Power Keeps Growing
Kim Ji Won has built a reputation for picking roles that connect with large audiences. Queen of Tears made her a household name across Asia, and her performance in My Liberation Notes earned her critical praise.
She spent time preparing for Doctor X by learning how to handle surgical instruments and practicing procedures under the guidance of a professional consultant. The actress said she wanted to show both sides of her character, a person with extraordinary abilities who struggles to express emotions.
With two episodes already above 11 percent and the second one approaching 15 percent, Doctor X is on a path that few Korean dramas reach this early in their run. The question now is how high the numbers will climb as the story unfolds.
