“I wanted to see someone die selflessly saving someone’s else’s life, and she was not only saving that little kid on the table, she was saving Jules,” Marinis said. “It really speaks to the kind of character Monica was.”

Reactions to the Character’s Exit

Natalie Morales, who had recurred as Beltran since season 20, was informed of the decision when she was asked to return for the new season. She described the news as a “mix of things,” expressing gratitude for her time on the show but also understanding the narrative reasoning.

“She’s a writer and director as well as being an actor. She understands story, and she just wanted to make sure that we made it the best story possible,” Marinis said about informing Morales of her character’s fate.

Morales approached her final scenes with a focus on medical accuracy, consulting with the show’s medical advisors to understand exactly how her character’s injuries would lead to her death. She even practiced her “death face” for the scene, though her final moments were filmed from a distance to preserve the surprise for the audience.

Fellow cast member Chris Carmack, whose character Link survived, shared his reaction to Morales’ departure. “I loved working with Natalie so much. She’s such a light, funny, wonderful, kind, great actress,” he said. “It definitely hurt to see that Beltran didn’t make it.”

Beltran’s death is set to have lasting effects on the other characters, particularly Dr. Jules Millin and Dr. Amelia Shepherd. Millin was deeply traumatized by the event, breaking down after informing her patient’s parents that their son was alive. For Amelia Shepherd, the loss is deeply personal. The two doctors had developed a close bond and a potential romantic relationship that was cut short. The showrunner confirmed that Amelia will grapple with guilt, feeling responsible for the chain of events that led to the explosion.

Beyond the personal loss, the hospital itself suffered massive damage. The OR floor was destroyed and may not be usable for weeks, forcing the staff to cancel all elective surgeries and improvise with temporary operating rooms.
