The hour focused on a medical mystery involving a mother and her two daughters who could not go outside in the sun. But the real story was inside Watson’s own head. His continued visions of Holmes ( Robert Carlyle ) became impossible to ignore, leading to a car accident cliffhanger that left viewers shocked.
A Medical Mystery That Hit Close to Home
The case of the episode featured two young sisters, Cora and Abby, who suffered from a rare condition. They could not tolerate sunlight at all. Their mother, Shelly, kept them hidden inside their home. She grew her own vegetables and controlled everything they ate.
The medical team first thought the mother might have mental problems. They believed she was keeping her children inside due to paranoid delusions. But Watson and his team dug deeper. They discovered the girls had a rare genetic condition made worse by eating too many celery and other vegetables high in certain chemicals.
The twist came when the mother dressed as a nurse to steal Cora from the hospital. Watson caught her within seconds. The case showed how easy it is to mistake a real medical condition for mental illness. This theme directly connected to Watson’s own struggles with his hallucinations.
Watson Faces the Hard Truth About Holmes
For several episodes, Watson has been seeing and talking to Sherlock Holmes. The show left viewers guessing whether Holmes was truly alive or just a figment of Watson’s imagination. Robert Carlyle, who plays Holmes, previously stated that as far as he is concerned, Holmes is very much alive. But Episode 16 suggested otherwise.
Watson kept seeing Holmes in mirrors and around corners. The hallucinations caused him to argue with thin air in front of other people. At one point, Holmes’ appearance made lights flicker on and off, making him seem more like a ghost.
The breaking point came when Watson was driving with his girlfriend Laila. Holmes appeared in the rearview mirror and distracted him. Watson swerved to avoid a deer and drove off an embankment. The crash served as the episode’s shocking ending. It forced Watson to realize that his mind has been playing tricks on him all along.
The Holmes Hallucination Storyline Divides Viewers
Not everyone likes where this story is going. Many fans have grown tired of the hallucination plot. One review called it “nonsense” and pointed out logical gaps. If Holmes is not real, how did Watson come home to find Holmes cooking him dinner?

Other Storylines Finally Get Attention
Episode 16 also brought back some dropped storylines from earlier in the season. Brenda returned after being absent for many episodes. The show also addressed Ingrid’s mental health struggles again. Ingrid had seemed stable for several weeks after losing her therapist. But this episode showed her becoming dysregulated again.
Ingrid started flirting with the idea of working with Beck again. She considered shading her statement in exchange for leading the therapy group. This plot would have made more sense if it happened right after Episode 11. The long gap between story beats made the character’s actions feel less consistent.
On a positive note, Ingrid’s visit to Beck paid off when she recognized a photo of Sasha’s mother. This discovery should lead to interesting developments in future episodes.
