Katey Sagal is coming back to the network that launched her career. The Married… With Children alum has joined the second season of Fox’s medical dramedy Best Medicine in a recurring role, the network confirmed this week. Sagal will play Kathleen Gavin, the estranged mother of schoolteacher Louisa Gavin (Abigail Spencer), a character the people of Port Wenn know by a far less flattering name.
The casting brings Sagal full circle with Fox, where she first became a household name as Peg Bundy on the long-running sitcom Married… With Children and later voiced Leela on the animated hit Futurama. Her arrival on Best Medicine adds a new layer of family tension to a show that has already built its second season around messy relationships and small-town chaos.
Who Is Kathleen Gavin on Best Medicine
Fox describes Kathleen Gavin in blunt terms. She is “Louisa’s estranged mother and the only person in the world who’s not welcome in Port Wenn.” The character is further described as “combative, conniving, and always looking out for herself”.
Kathleen returns to town after a long absence, and her reappearance sends Louisa’s life into a tailspin. Dr. Martin Best (Josh Charles) quickly learns there is a solid reason the locals call her “Shady Katie.” The nickname alone tells viewers everything they need to know about how Port Wenn feels about Louisa’s mother.
Sagal’s episodes are set to air later in the current second season. Exact episode counts and air dates for her arc have not been announced yet. Her first appearance will come after the show’s November 3 episode, which is the sixth installment of the season.
A Full-Circle Return to Fox for Katey Sagal
Sagal’s history with Fox runs deep. Married… With Children ran for 11 seasons on the network before ending in 1997, and the show was a foundational piece of Fox’s early primetime lineup. She later lent her voice to Futurama, which originally aired on Fox from 1999 to 2003 before finding new life in revivals elsewhere.
Her recent television work includes roles on The Conners, Rebel, The Abandons, and One Piece. She won a Golden Globe for her performance on Sons of Anarchy and has also starred in 8 Simple Rules. Sagal is repped by CAA and B&B Management.
The return to Fox is not just a nostalgic booking. Best Medicine has become one of the network’s stronger performers, and adding a familiar face with Sagal’s range gives the second season a new conflict to play with.
What Best Medicine Is About
Best Medicine is an American adaptation of the long-running British series Doc Martin, created by Dominic Minghella. The Fox version was developed by Liz Tuccillo and premiered on January 4, 2026.
The story follows Dr. Martin Best, a brilliant Boston surgeon who leaves his big-city career to become the general practitioner in the small fishing village of Port Wenn, Maine, where he spent summers as a child. Martin is gifted at diagnosing unusual medical conditions but struggles with people. His blunt bedside manner rubs the quirky townsfolk the wrong way, including Louisa, the local schoolteacher.
Martin also carries a secret that complicates his work: a blood phobia that ended his surgical career. The show balances his medical mysteries with his slow, awkward attempts to connect with the people around him.
The main cast includes Josh Charles as Dr. Martin Best, Abigail Spencer as Louisa Gavin, Annie Potts as Aunt Sarah, Josh Segarra as Sheriff Mark Mylow, and Cree Cicchino as Elaine Denton. Season 2 also brought in Jack Cutmore-Scott as Parker, a potential romantic interest for Louisa, and Andrea Martin as an eccentric friend of Aunt Sarah. Martin Clunes, the star of the original Doc Martin, also appears in the second season as Martin’s father.
How Best Medicine Season 2 Is Performing and When New Episodes Air
Fox renewed Best Medicine for a second season in March 2026, before the first season had finished airing. The first season’s premiere reached 12.6 million multiplatform viewers, and when Season 1 arrived on Netflix in August 2026, it passed 1.6 billion viewing minutes.
Season 2 premiered with a special preview episode on Sunday, September 20, 2026, followed by its regular Tuesday time slot premiere on September 22. The season was originally ordered for 12 episodes but was later increased to 14 episodes.
New episodes air Tuesdays at 8 p.m. ET/PT on Fox. Episodes stream the next day on Hulu. For viewers outside the United States, Best Medicine is a Fox network series, and international availability depends on local licensing deals. The show’s first season is also available on Netflix in several markets.
The next new episode, “Can’t Stop the Healing,” is scheduled for Tuesday, November 3, 2026, on Fox. Sagal’s episodes will arrive later in the season.
Why Louisa’s Mother Matters to the Story
Louisa Gavin has been a central figure in Best Medicine since the beginning. She is the town’s schoolteacher, Martin’s on-and-off romantic interest, and one of the few people who can push back against his abrasive personality.
Bringing in Louisa’s estranged mother gives the show a chance to explore a side of Louisa that viewers have not seen yet. Kathleen is not a warm, welcoming presence. The official description makes it clear she is a disruptor, and her return is designed to throw Louisa’s personal life into disorder.
The timing also matters. Season 2 has already introduced Parker as a new romantic possibility for Louisa, and Martin’s feelings for her remain a running thread. Kathleen’s arrival adds another variable to an already complicated situation.

Katey Sagal’s Role in the Show’s Second Season
Sagal is the latest in a series of casting additions for Season 2. Martin Clunes, the original star of Doc Martin, returned to the franchise in a different role as Martin’s father. Jack Cutmore-Scott and Andrea Martin also joined the season.
For Sagal, the role is a return to the network where she built her career, but it is also a chance to play a character described in unusually sharp terms. Kathleen is not written as a likable figure. She is written as a problem, and the show is positioning her as a source of conflict for Louisa and Martin alike.
Best Medicine airs Tuesdays at 8 p.m. ET/PT on Fox, with next-day streaming on Hulu. The November 3 episode is the next new installment before Sagal’s arc begins later in the season.
