Doctor X: Mafia in White has quickly become one of the most talked-about Korean dramas of 2026. The SBS medical noir, starring Kim Ji-won in her first small-screen role since Queen of Tears, follows a brilliant surgeon who refuses to bow to hospital politics. Episodes 1 to 4 have already delivered tense surgeries, bitter power struggles and a growing mystery about the heroine’s past. This recap breaks down exactly where each major character stands after the first four episodes.

Gye Su-jeong: The Genius Surgeon Who Refuses to Play by Hospital Rules

Gye Su-jeong (Kim Ji-won) arrives at Guseo University Hospital through a doctor placement agency run by Jang Hui-suk. From her first day, she makes it clear that she has no interest in hospital hierarchy or flattery. She moves a supposedly paralysed VVIP out of his wheelchair to make room for a critically ill patient, exposing the VIP as a fraud in the process. When a complicated surgery planned by senior doctors goes wrong, she steps in and saves the patient, leaving the staff stunned by her skill.

In episode 2, the hospital faces a brutal choice between two pancreatic cancer patients: a wealthy chairman and major donor, and a young diabetic girl. Su-jeong refuses to let money decide who lives. She operates on the girl first and then takes over the chairman’s surgery from Do Yun-sik, completing both operations successfully. Her confidence is absolute. When a patient hesitates about a risky operation, Su-jeong tells her: “Without surgery, you will die within a month. With surgery, you will still die someday, but if I operate, it will be a hundred years from now”.

Episode 3 puts her career on the line. She performs emergency brain surgery on a hospital cleaner without waiting for official approval. The senior doctors are furious, and her contract is placed on hold. She is moved to the emergency room while the hospital hires a new general surgeon. In episode 4, she continues to work in the ER and even steps into another doctor’s surgery to help locate a wound. She remains unfazed by the hostility around her.

Jang Hui-suk: The Agency Head Who Protects Her Top Asset

Jang Hui-suk (Lee Jung-eun) runs the doctor staffing agency that places Su-jeong at Guseo University Hospital. She appears materialistic and talkative, but she is a shrewd operator who understands exactly how much Su-jeong is worth. When the hospital puts Su-jeong’s contract on hold in episode 4, Hui-suk threatens to sue Director Bu if he does not pay what is owed. She also takes Su-jeong out for a meal and tells her to hold her head high despite being assigned to the ER.

Hui-suk shares a deeper bond with Su-jeong than a simple business relationship. Flashbacks in episodes 1, 2 and 4 show the two women connected to a fire at a White Wings Medical Group facility overseas. Hui-suk was present when Su-jeong tried and failed to revive a boy named Baraka after the fire. The exact nature of their past and why they now work in the medical staffing business remains one of the show’s biggest questions.

Bu Seung-kwon: The Hospital Director Walking a Thin Line

Bu Seung-kwon (Son Hyun-joo) is the director of Guseo University Hospital, and his position is far from simple. He is caught between the hospital’s powerful backers and his own sense of what is right. In episode 4, he faces backlash from management after holding a press conference that blamed the previous director for the death of a doctor named Seo Jin-yeong.

Bu’s relationship with Su-jeong is equally complicated. He recognises her talent and does not fire her, even when the senior doctors demand it. When Bae Heung-gon warns him that Su-jeong is the mysterious Doctor X from a seven-year-old case, Bu’s response is unexpected. He says that if that is true, it is all the more reason to keep her at the hospital. Whether Bu is a protector, a strategist or something darker has not yet been revealed.

Park Tae-kyung: The Intern Who Sees Another Side of Su-jeong

Park Tae-kyung (Kim Woo-seok) is a new intern at Guseo University Hospital. He begins the series with a bright, earnest respect for senior doctors, but his view of the profession is tested by the hospital’s culture. Tae-kyung is initially suspicious of Su-jeong. In episode 2, when she leaves the girl’s surgery midway to handle the chairman’s operation, he assumes she is abandoning the patient for money and status. He follows her and buys her coffee, only to realise she had already completed the girl’s surgery flawlessly.

Tae-kyung works late and pushes himself hard. By episode 4, he suffers a nosebleed from exhaustion and begins to question whether surgery is a rewarding path. He is one of the few younger doctors who is genuinely trying to understand Su-jeong rather than simply oppose her.

Ma Seong-gyu and the Old Guard: Doctors Threatened by Doctor X

Ma Seong-gyu (Kwon Seung-woo) is a senior resident who represents the hospital’s old guard. He acknowledges Su-jeong’s surgical skill but deeply disapproves of her methods and attitude. The seven-year-old medical report from Cuba becomes his main weapon against her. The report describes a high-risk operation performed by a surgeon known only as Doctor X, and the patient died two months later. Seong-gyu uses this article to argue that Su-jeong will do anything to ensure a surgery’s success, regardless of the patient’s safety.

Bae Heung-gon (Lee Sung-wook) is another senior doctor who suspects Su-jeong is Doctor X. He shows the old article to Director Bu and insists that if the same thing happens to the chairman, the hospital will be held responsible. The old guard’s resistance to Su-jeong is not just about her attitude. It is about a system that rewards seniority and connections over actual skill.

Na Dae-il: The New Surgeon With a Suspicious Interest in Su-jeong

Na Dae-il (Jang Seung-jo) arrives in episode 4 as the new general surgeon hired while Su-jeong’s contract is on hold. He is charming and good-looking, and the other doctors are quickly impressed by him. His interest in Su-jeong, however, is immediate and unsettling. He calls her “Snake Su-jeong” and asks Park Tae-kyung to help him pursue her. During a surgery, he grabs her hand inappropriately while she is trying to locate a wound inside a patient’s body.

Dae-il’s connection to the hospital goes deeper than a new job. He is a former military surgeon, and he shares a past with a patient named Kwon, whom he saved during his service. His true intentions and why he is so fixated on Su-jeong remain unclear, but he is clearly being set up as a new source of conflict.

The Fire Mystery and the Doctor X Legend

The flashbacks to the fire at the White Wings Medical Group facility are the emotional backbone of the first four episodes. In episode 1, the show opens with a burning building and a young Su-jeong running into the flames. Episode 2 reveals that she promised a little girl she would save her brother. Episode 4 shows the aftermath: Su-jeong tries to revive a boy named Baraka, but he dies from his burns. The mystery of what exactly happened that night, and how it connects to the seven-year-old Doctor X case in Cuba, is the thread that ties Su-jeong’s past to her present battle at Guseo University Hospital.
