Doc Season 3 continued its weekly run on Fox with Episode 5, titled “S.O.S,” which aired on Tuesday, October 20, 2026, at 9 p.m. ET/PT. The episode placed TJ and Sonya’s new relationship under real strain for the first time while Amy treated a young patient whose illness pulled her back into the grief of losing her son, Danny. The hour also moved forward with Gina and Wendy’s family plans and introduced friction around the new intern Graham.
What Happened Before Episode 5
Episode 4, “Great Expectations,” ended with Sonya telling TJ she was seven weeks pregnant. TJ responded with excitement and promised to be there for her, which seemed to settle the couple’s first major challenge. Amy spent the episode clashing with Dr. Ben Grant over a patient tied to her past, while flashbacks showed her romantic history with Ben at medical conferences. Gina and Wendy were already exploring options for starting a family, and the hospital was adjusting to a new class of interns, including the well-connected Graham.
TJ Pulls Away From Sonya
The pregnancy news did not lead to the smooth path TJ promised. TJ left Sonya’s place while she was sleeping and sent a text instead of staying to talk. Sonya told Amy she felt TJ was “detached” from the situation. TJ brushed off Amy’s concerns, but Amy’s memory flashes revealed a pattern: TJ avoids difficult situations.
The episode also showed how severe TJ’s PTSD has been. A flashback revealed a nightmare so intense that TJ physically attacked Amy in his sleep, slamming her against a wall and choking her before he woke up. Amy had told him then that he needed help. TJ claimed in the present that he had spoken to someone, but the episode left doubt around that claim.
“TJ is genuinely, in my opinion, extremely excited about having a child. It is really scary with the aspirations that he has in life. And he’s growing and excelling extremely fast.” — Patrick Walker, who plays TJ, told TV Insider.
By the end of the episode, TJ told Sonya he needed space for the night to get some sleep. Sonya was left hurt and confused because she does not know what TJ is carrying.
Amy’s Patient and the Danny Memories
Amy treated a young patient named Roxy who developed subacute sclerosing panencephalitis, or SSPE, a rare complication of measles that can appear years after the original infection. Roxy had been exposed to measles through an unvaccinated relative. The case carried fatal stakes and triggered Amy’s memories of losing her own son, Danny.
Amy’s grief led to another confrontation with Michael, and their argument showed how differently the two have carried the same loss. Michael remembers Danny’s death. Amy is only now beginning to remember it again.
Jake, Amie, and the Distance Between Them
Jake had a difficult hour. His attitude toward Amie drew criticism, with Jake seeming to want Amie to understand his hurt while offering little understanding of hers. Amie is still piecing together who she is while parts of her past return without warning.
“There is a difference between needing distance and making every interaction feel like a punishment.” — Fangirlish review of the episode.
Gina and Wendy Choose a Donor
Gina and Wendy made a major decision about their path to parenthood. The couple chose Wendy’s brother as their sperm donor so that their child would share a biological connection with both of them. The choice came with complications around boundaries and family expectations.
Graham’s Rough Start at Westside
New intern Graham Barker continued to lean on his family connections. He repeatedly told Amy that his stepmother is a huge fan of hers, but Amy was not impressed by the flattery. Graham also used Kyle throughout the hour, dangling the prospect of friendship and a baseball game over him to get what he needed. The episode framed Graham as entitled and polished, a character aware of the privilege attached to his name.
The Mason Case
TJ and Sonya worked together on a patient named Mason, whose injuries appeared to be self-inflicted. Mason had damaged his own face in an attempt to reshape his jawline, a practice tied to online appearance culture. A flesh-eating infection from bone fragments cost him half of his jaw, and he faced years of further surgeries. The case kept TJ and Sonya in the same room even as their personal relationship grew more uncertain.
