Chicago Med is heading into its twelfth season with two major cast changes that could not have come at a trickier moment for the NBC medical drama. Sarah Ramos, who plays Dr. Caitlin Lenox, will be absent for the first five episodes of Season 12 due to maternity leave, while Ashlei Sharpe Chestnut, who portrays Dr. Naomi Howard, has confirmed she will not return at all when the show comes back on Wednesday, October 7, 2026 at 8 p.m. ET on NBC.
Sarah Ramos Missing Five Episodes During Key Storyline
Sarah Ramos announced her pregnancy in May 2026 before the Season 11 finale aired. Showrunner Allen MacDonald confirmed that the series will not write the pregnancy into the show, meaning Lenox will simply be away from Gaffney Medical Center for a stretch of early episodes.
“Sarah, because of her pregnancy, will be taking the majority of the beginning of the season off. She will be away for a while.”
The timing is rough because Dr. Lenox was in the middle of one of the show’s most important arcs. During Season 11, viewers watched Lenox deal with a diagnosis of Gerstmann-Sträussler-Scheinker (GSS) disease, a rare and debilitating condition. Her relationship with Dr. Mitch Ripley (Luke Mitchell) also reached a new level when the two finally confessed their feelings for each other. Now fans will have to wait at least five episodes to see how Lenox is doing and where her story goes next.
Ramos is confirmed to return as a series regular later in the season, so this is a temporary absence rather than a permanent exit.
Ashlei Sharpe Chestnut Confirms Permanent Exit as Dr. Naomi Howard
The bigger loss for the show is Ashlei Sharpe Chestnut, who has played Dr. Naomi Howard since Season 9. A fan asked her directly on TikTok in September 2026 whether she would be back for Season 12. Her reply was short and clear: “Thank you! Hope you’re well too:) And no.”
Chestnut’s exit follows a confusing few years for her character. Naomi appeared in Seasons 9 and 10, then seemed to disappear, leading many viewers to believe she had already been written off. She returned in the Season 11 premiere, and MacDonald told Deadline in October 2025 that the show never planned to write her out.
“We love Ashlei Sharpe Chesnut; she’s phenomenal. We never intended to write her out.”
Despite that statement, Chestnut will not be part of Season 12. Her character was last seen in a relationship with Dr. John Frost (Darren Barnet), so fans will now wonder how that storyline will be handled without her on screen.
Season 12 Storylines Face Major Adjustments
The departures create real challenges for Season 12, which MacDonald has described as being about “new beginnings”. Two of the show’s romantic pairings will be affected. Lenox and Ripley’s relationship was just starting to settle into a good place, and Frost and Naomi were building something meaningful. Now both stories face uncertain paths.
Other Season 12 storylines include a major power struggle between Sharon Goodwin (S. Epatha Merkerson) and board member Miranda Lewis (Orlagh Cassidy). The Season 11 finale ended with Lewis accusing Goodwin of ethical breaches and demanding her resignation.
“Next season, I think that we’re gonna basically see Miranda and Goodwin get in the ring. Goodwin is going to have to fight to keep her job.”
Dr. Dean Archer (Steven Weber) and Dr. Hannah Asher (Jessy Schram) are also navigating life as new parents to their daughter, Mabel, while living together.
One Chicago Franchise Sees Multiple Cast Changes
The cast shake-ups are not limited to Chicago Med. Joe Miñoso is leaving Chicago Fire after playing firefighter Joe Cruz since the show began in 2012, though he will appear in several episodes of Season 15. LaRoyce Hawkins is departing Chicago P.D. after 13 seasons as Kevin Atwater. Dermot Mulroney is also wrapping up his storyline on Chicago Fire.
NBC’s president of program planning Jeff Bader addressed the franchise’s frequent cast changes in July 2024, saying, “Cast comes in, cast goes out… those shows just keep going on.”
