Chicago Med returned for its 12th season on Wednesday, October 7, 2026, with a premiere episode titled “Shots Fired” that threw Gaffney Chicago Medical Center into a mass casualty crisis. The episode introduced two new characters, brought back familiar faces, and featured a guest appearance from a Beverly Hills, 90210 alum. New Faces at Gaffney

An-Li Bogan joined the cast as Dr. Annie Wu, a brilliant but headstrong resident introduced in the emergency department under Dr. Mitch Ripley’s mentorship. Bogan is known for her roles in series like The Unsettling, Bull, Elementary, and Love Life, along with films such as After Yang and The Plan. Dr. Wu arrives as a new doctor who does not even know who Sharon Goodwin is, and her quick thinking and ambition create tension during high-pressure cases. Showrunner Allen MacDonald described her as “a brilliant but morally unpredictable physician” who will become highly competitive with Dr. Frost.

“She comes in as someone who’s very ambitious and has an edge to her that I don’t think we’ve necessarily seen yet.”

Rebecca Breeds joined as Mallory Sloan, the interim executive director who stepped in while Sharon Goodwin is on voluntary leave. Breeds rose to fame through Australian shows like Home and Away and Blue Water High, and later starred as Clarice Starling in the series Clarice. She has also appeared in Legacies, Pretty Little Liars, and The Originals. Sloan’s focus on budget cuts and hospital finances immediately put her at odds with Dr. Dean Archer, creating a sharp contrast with Goodwin’s leadership style.

Returning Main Cast

S. Epatha Merkerson returned as Sharon Goodwin, the longtime director of patient and medical services at Gaffney. Goodwin spent the premiere fighting to clear her name after board member Miranda Lewis (Orlagh Cassidy) accused her of leaking Dr. Theo Rabari’s private medical files. She took a voluntary paid leave pending the investigation. The episode ended with a major twist when Nurse Doris (Lorena Diaz) arrived at Goodwin’s home and confessed to the leak.

Steven Weber returned as Dr. Dean Archer, the co-director of the emergency department. Archer has become a father to baby Mabel with Dr. Hannah Asher, and the premiere showed them navigating their undefined relationship while caring for their daughter. Archer also clashed with Sloan over staffing shortages in the emergency department.

Jessy Schram returned as Dr. Hannah Asher, an OB/GYN who was on maternity leave during the premiere. She visited the hospital with baby Mabel and brought food for Archer. When the mass shooting victims arrived, Asher stepped in to lead the surgery on a pregnant bride. She confirmed to colleagues that she and Archer are simply co-parenting, though the spark between them was clear to everyone around them.

Luke Mitchell returned as Dr. Mitch Ripley, an emergency department physician who was mentoring the new resident Dr. Annie Wu. Ripley also dealt with a personal connection to the shooter when a car crashed through the emergency department doors. His relationship with Dr. Caitlin Lenox continued through video chats while she is in Zurich participating in a prion drug trial.

Sarah Ramos appeared as Dr. Caitlin Lenox in a limited capacity. Ramos is on maternity leave, and her character is written as being in Zurich for a medical trial. Lenox and Ripley are making their long-distance relationship work while she receives treatment for the terminal prion disease she was diagnosed with in Season 11.

Oliver Platt returned as Dr. Daniel Charles, the chief of psychiatry at Gaffney. Charles offered to help the emergency department during the staffing crisis and provided guidance to the groom, Dexter, who admitted he had doubts before the wedding. Charles also appeared to be in a much better place compared to his struggles in Season 11.

Darren Barnet returned as Dr. John Frost, a pediatric resident who treated an 8-year-old patient named Dakota who lost her sight after the shooting. Frost ordered tests that revealed her blindness was a trauma response.

Guest Stars in the Premiere

Gabrielle Carteris guest starred as Birdie Sutherland, the mother of the groom, Dexter. Carteris is best known for playing Andrea Zuckerman on Beverly Hills, 90210. Showrunner Allen MacDonald described her character as “an overbearing mother with a very complicated relationship with her son.” Carteris teased her involvement on social media in August 2026.

Scarlett Spears guest starred as Dakota Cross, the 8-year-old patient who lost her sight. Spears is known for her role on General Hospital.

Jimi Stanton played Dexter Sutherland, the groom who went missing during the shooting. Dexter eventually arrived at the hospital uninjured and admitted he never drove to the church because he was unsure about the wedding.

Michael Alix played Bruno, a wedding musician who brought Dakota to the hospital. Bruno encouraged Dakota throughout her treatment and promised to return to check on her, but he was killed when the shooter’s car crashed through the emergency department doors.

Shani Atias appeared as Violet, a woman who arrived at the hospital unconscious and in life-threatening condition with suspected meningitis.
