Best Medicine returned for its second season on Fox with a premiere that delivered everything fans love about the quirky medical dramedy: small-town chaos, a bizarre medical mystery, and a major secret finally coming to light. The episode, titled “Be Careful What You Fish For,” aired on September 20, 2026, and picked up months after the events of the Season 1 finale. At the center of it all was Dr. Martin Best (Josh Charles), who found himself facing his greatest fear in front of the entire town of Port Wenn, Maine.
The premiere balanced multiple storylines, from a woman dressed as a mermaid washing ashore to a chain-smoking houseguest causing explosions, all while Martin’s professional future hung in the balance. But the hour’s most significant moment came when Martin willingly revealed the secret that had haunted him since the series began.
Martin Faces the Medical Board Monitor
The episode opened with Martin still under investigation by the Maine State Medical Board following the unlicensed blood draw scandal from Season 1. A new face arrived in Port Wenn: Judith, played by guest star Andrea Martin. Judith is a friend of Sarah’s (Annie Potts) who has just been released from the hospital after treatment for bilateral pneumonia. She now relies on prescribed oxygen, but she cannot stop smoking.
Her stubborn habit leads to disaster. Judith causes an explosion first at her own home and then at Sarah’s house, leaving Martin furious. Sarah, however, scolds her nephew for failing to understand that looking after neighbors, no matter how frustrating they may be, is part of living in a community. The conflict highlights the ongoing tension between Martin’s instinct to isolate and the town’s expectation that he engage with the people around him.
A Mermaid Washes Ashore in Port Wenn
The case of the week arrived in spectacular fashion. While helping Sarah near the shore, Martin spotted a woman dressed as a mermaid struggling near a buoy. He dove into the water and swam out to rescue her.
The woman, whose name was later revealed as Cecilia, could not speak clearly at first and suffered a seizure. The cause was eventually identified as a toxic algae bloom in the water. Cecilia also has a skin condition that causes thick, scaly skin on her legs, something she has spent her life trying to hide.
At first, Cecilia claimed she was a cruise ship performer who fell overboard. But Mark later discovered a missing person report for her, prompting Cecilia to admit she had been on a party boat with friends. She drank too much, and none of her friends noticed she was gone. Martin feared she may have jumped intentionally, and the situation escalated when Cecilia climbed to the top of the lighthouse.
The Town Confesses as Martin Reveals His Secret
With the whole town gathered at the base of the lighthouse, residents tried to talk Cecilia down. They began shouting their own embarrassing truths, hoping to show her that everyone has something they are ashamed of. Louisa admitted she worried she might never find love again.
In that moment, Martin made a decision that changed everything. He yelled his guarded secret in front of the entire town: “I have a blood phobia!”.
The confession was not forced by Glendon’s threat or the medical board investigation. Martin chose to reveal it himself, in service of helping a patient who was struggling. The showrunners explained the significance of this choice in an interview with TV Insider.
“Martin reveals his blood phobia and the way it’s done, it’s clearly something that he’s deciding to do. He’s thinking about it as much as he can in that moment as everyone else is confessing things,” the bosses said.
Most of the townspeople did not seem to care about Martin’s phobia, which was a surprisingly warm response. Sarah, however, was hurt that Martin told the whole town before telling her. The reveal also canceled out Glendon’s threat to expose him, shifting the power dynamic that had defined Martin’s secret since the series began.
Greg and George Face the Sea Urchin Boom
The premiere also checked in with Greg and George, the owners of the Salty Breeze, who have been promoted to series regulars. Their success from the local sea urchin boom has started to take a toll on both of them. The storyline adds to the ensemble feel of the episode, showing that the changes in Port Wenn affect everyone, not just Martin.
