Kim Ji-won returns as the surgeon everyone loves to doubt in Doctor X: Mafia in White, and this time the stakes are almost impossible to believe. In the newest episode of the SBS Friday-Saturday drama, her character Gye Soo-jung performs a brain surgery to remove a bullet fragment that has been stuck inside a patientโs head for 46 years. The procedure carries a success rate so low that every other surgeon at Guseo University Hospital refused to touch it. The operation is not happening in private, either. Hospital staff, senior doctors, and outside observers are all watching as Kim Ji-wonโs character tries to do what no one else would even attempt.
The episode airs on October 30 at 9:50 p.m. KST on SBS, and it is already the most talked-about hour of television in South Korea this week.
The Patient Everyone Else Gave Up On
The surgery centers on a man named Oh Sang-beom, played by Ha Sung-kwang. His case is not just medically difficult. It is a public spectacle. The dramaโs previous episode established that Oh Sang-beom has been living with a bullet fragment lodged in his brain for 46 years, and the entire country has been paying attention to his situation. The extreme risk of the operation, combined with the nationwide interest, made every other surgeon at Guseo University Hospital step back.
Gye Soo-jung did not step back.
She volunteered.
The decision fits the character viewers have come to know across the first six episodes: a freelance surgeon who works through a staffing agency, takes a flat fee per operation, and refuses to play by the hospitalโs unspoken rules of hierarchy and self-protection. Kim Ji-wonโs character is nicknamed the โmad dog of the operating roomโ because she chases the hardest cases with a focus that unsettles the people around her.
The surgery itself comes with two nightmare outcomes that the show has not softened. The patient could die on the operating table, a risk the drama refers to as โtable death.โ Or he could survive in a vegetative state. Neither possibility is hidden from the audience or from the characters in the room.
A Surgeon Who Does Not Blink While Everyone Watches
New stills released by the production team show Gye Soo-jung in the operating room with her hands steady and her eyes fixed on the surgical field. Around her, the room is crowded with people who are not there to help. Hospital director Boo Seung-kwon, played by Son Hyun-joo, stands among the observers. Veteran doctors with decades of combined experience are also present, and none of them can look away.
The tension in the scene comes from the contrast between the crowdโs visible fear and Kim Ji-wonโs stillness. The characterโs personal rule is simple, and the drama has repeated it across promotional material and previews: โI do not fail.โ That line is not a boast in the context of the show. It is the only thing she has to hold onto in a room full of people waiting for her to make a mistake.
โEither table death or a vegetative state. If itโs not about forcing someone to take on a surgery with no guarantee of the patient walking out intact, then what is it?โ โ Professor Hwang, played by Kang Hak Su, during the surgery conference
โWhy should we perform a surgery that everyone will just sit back and watch? Whoโs going to do it?!โ โ Professor Jang, played by Im Cheol Hyeong, voicing the hospitalโs resistance
Gye Soo-jung answered that question by raising her hand. In the preview footage, she bursts into the conference hall with her face flushed and says, โMe! Iโll perform the surgery!โ.
The Question the Show Has Not Answered Yet
The surgery is not only a medical event. It is also a mystery.
In the previous episode, while reviewing Oh Sang-beomโs brain CT scan, Gye Soo-jung noticed something unusual. She murmured a question that the drama has not resolved: โWhat on earth happened that day?โ. The preview materials for episode 7 keep that question open. The bullet is old, but the story behind it is not fully explained. The surgery is therefore both a rescue mission and an investigation, and the patientโs life depends on Kim Ji-wonโs character figuring out what the scan is trying to tell her before it is too late.
That unresolved detail is part of why the episode has drawn so much attention. The drama has built its first six episodes around Gye Soo-jung uncovering corruption at Guseo University Hospital, and the bullet case connects directly to the larger question of who has been protected and who has been sacrificed inside the institution.
The Ratings Are Already Historic
Doctor X: Mafia in White is not a small show. It is the biggest drama on South Korean television right now, and the numbers back that up.
By episode 6, the drama recorded a peak viewership rating of 21.3% and a nationwide average of 19.5%, according to Nielsen Korea. Those figures placed it fifth among the highest-rated SBS Friday-Saturday dramas of all time, and it reached that position in only six episodes.
The premiere episode alone scored 11.4%, which was the highest debut rating for any South Korean miniseries in 2026. The show has broken its own record almost every week since.
On the global side, Netflix reported that Doctor X: Mafia in White ranked second on its weekly Global Top 10 Non-English Shows chart, holding a top-tier position from its first week through its third week of release. The series accumulated 36.7 million cumulative views during that period, even while competing with major international releases.
Kim Ji-won herself has ranked No. 1 in the TV-OTT integrated cast buzzworthiness category for three consecutive weeks, according to Good Data Corporationโs FUNdex. Lee Jung-eun, Son Hyun-joo, and Kim Woo-seok also ranked in the upper tiers of drama performer rankings.
A Gallup Korea poll released on October 28 named Doctor X the most popular show of October 2026, with a 9.8% approval rating. It is one of only seven SBS dramas to top that monthly survey since the poll began in 2013.
Where to Watch and When

The episode will answer one question that every character in that operating room is asking: whether Gye Soo-jung can save a man that the entire hospital had already given up on. The answer is not in the previews. It is in the surgery itself, and Kim Ji-wonโs character is the only one willing to find out.
