The medical drama Doc finished its second season with a two-hour event that left viewers processing a lot of information. The episode, which aired on Tuesday, April 14, 2026, on FOX, did not hold back on drama. From a deadly virus lockdown to a major character death and a secret romance from the past, the finale packed in many twists. The biggest surprise came in the final minutes, revealing that Dr. Amy Larsen (Molly Parker) had a hidden relationship with a new surgeon, Dr. Ben Grant (Blair Underwood).
A Deadly Marburg Virus Locks Down Westside Hospital
The finale begins with a normal day that quickly turns dangerous. A patient dies from a mysterious illness. Dr. Amy Larsen discovers it is a highly contagious strain of the Marburg virus. The hospital immediately locks down the sixth floor to stop an outbreak.
The situation brings back painful memories for Amy. She experiences intense flashbacks to the early days of the COVID-19 pandemic. These memories overwhelm her, but she tries to hide her struggle from her boyfriend, Jake (Jon Ecker). While the staff fights to contain the virus, several key characters get trapped in quarantine, including Richard (Scott Wolf) and Gina.
Richard ends up volunteering to test a new treatment. The risky procedure works, but not before his heart rate drops to dangerous levels. His blood becomes the key to creating a cure. Unfortunately, the supply is very limited. The hospital can only save three people. While trying to save others, Amy accidentally pricks herself with a contaminated needle. Her condition worsens quickly, forcing the team into an emergency surgery to save her life.
Amy Makes a Big Decision About Michael and Jake
The love triangle between Amy, her ex-husband Michael (Omar Metwally), and her current partner Jake has been a major part of Season 2. The finale addresses this head-on. While in quarantine, Jake shares an emotional moment with his ex-wife, Rachel. Amy watches them from a distance and realizes that Jake still has strong feelings for his past.
After surviving her surgery, Amy decides to step away from both men. She tells Michael she cannot live in the past. She also realizes that she and Jake cannot move forward when they both have feelings for other people.
“She’s raised some important issues that [she and Jake] have, and when she asks him, ‘You said we could deal with the fact that we have feelings for these other people, but what if you were wrong?’ and he doesn’t have much of an answer, that tells you they’ve got some stuff to figure out.” – Co-showrunner Hank Steinberg
Amy chooses to focus entirely on her job. She decides to dedicate her life to being a doctor and a teacher, putting romance aside for the time being.
Joan’s Heroic Goodbye
Dr. Joan Ridley (Felicity Huffman) has been battling a terminal illness throughout the season. Despite being weak, she refuses to rest during the virus crisis. She helps with contact tracing and eventually joins the operating team to save Amy’s life.
The surgery saves Amy, but the effort is too much for Joan’s body. The finale shows Joan returning home to hospice care. She dies peacefully with her family by her side. Her final act was giving her life to save her student.
Felicity Huffman explained that this ending was always the plan for her character. Joan went out in the most heroic way possible, sacrificing herself for someone she believed in.
The Affair Twist with Dr. Ben Grant
The most talked-about moment of the finale involves Blair Underwood’s character, Dr. Ben Grant. He arrives at Westside as a skilled cardiothoracic surgeon. When the virus hits, he steps in to help with surgeries. He tells Amy they met briefly at a conference three years ago. Since Amy lost her memory of the last eight years, she does not remember him.
However, the final scene reveals the truth. Ben watches a video message on his phone. The video is from Amy in November 2022. In the video, a past Amy wishes him a happy birthday. She admits they are having a secret affair. She says she is “breaking the rule” by recording the message because they usually never put anything in writing.
“I know we never text or put anything in writing, but I’m breaking the rule just this once, because it’s your birthday. But since we can’t be in that great suite in the Four Seasons to celebrate properly, I am making you this ridiculous video. I don’t know what this is or what we are, but I do know that you make me smile, and I haven’t smiled in a very long time.” – Past Amy in the video message
This twist changes everything. Ben knows about their romantic past, but Amy has no idea. He will remember every detail of their relationship while she sees him as a complete stranger.

What This Means for Season 3
Blair Underwood is set to join the cast as a series regular for Season 3. The show’s creators have confirmed they planned this reveal from the beginning. They wanted to introduce someone who has a history with Amy that even her best friend Gina does not know about.
Co-showrunner Hank Steinberg says he wants the audience to question Ben’s trustworthiness. He knows their secret but has not told her yet. This dynamic will bring a “very different energy” to the show when it returns. The stage is set for a complicated reunion. Amy has sworn off romance to focus on work, but her secret past lover is now her new colleague.
