The latest episode of Doc delivered major changes for the staff at Westside Hospital. The March 31, 2026, episode, titled “Next,” saw Dr. Joan Ridley (Felicity Huffman) make her final decisions before leaving the hospital, creating a new leadership structure that will shape the remaining episodes of Season 2.
The episode also pushed Dr. Amy Larsen (Molly Parker) and Dr. Jake Heller (Jon Ecker) to a difficult place in their relationship. Their tension while treating a pregnant teenager brought long-simmering issues to the surface, leaving fans questioning if the couple can recover.
Joan’s Final Day Brings Big Decisions for Westside’s Future
Joan Ridley spent her last day at Westside Hospital making two major choices that will affect the staff for the rest of the season. After agreeing to take the chief position with veto power over her replacement, Joan met with candidates throughout the episode with Dr. Michael Hamda (Omar Metwally) by her side.
Joan found reasons to dislike each candidate until Michael finally asked the question: “What about me?” This forced Joan to see what had been in front of her all along. She made Michael interview for the position, ensuring he understood the responsibility. Michael then sold the idea to Max (Darrin Baker), pointing out that Max could focus on fundraising and hospital image while Michael returned to saving patients and training doctors.
Felicity Huffman explained Joan’s thought process about Michael in an interview.
“I don’t even think until that final scene where he says, ‘What about me?’ that she even considers him because Michael’s great behind a desk, and the chief of the IM certainly has desk duties, but they’re really the coach of the whole team. And I didn’t know if she thought of him as that. I don’t even know that she thought of him as a doctor because when he was married to Amy, she so overshadowed him with her talent.”
The actress also noted that Joan sees this as an interim solution. “It’s a perfect solution for this interim period until hopefully Amy gets on her feet and becomes the chief again,” Huffman added.
Co-Chief Residents Create New Dynamic at the Hospital
The chief resident decision proved more complicated. The position came down to Amy and Dr. Sonya Maitra (Anya Banerjee). Joan met with Sonya first, and Sonya refused to disparage Amy to build her case. She told Joan she wanted to earn the position on her own merits.
When Joan spoke with Amy, she already knew Michael would replace her. Joan warned Amy not to think this meant there would be two chiefs in the family. Huffman revealed that the showrunners initially planned to give the position only to Sonya. “I went, ‘How can that possibly be? Because at every corner, we’ve seen that Amy outperforms Sonya,’” Huffman said. The idea of making them co-chiefs came from recognizing what each character brings to the role.
“The chief resident needs is to be a great teacher, and a great mentor, and a wonderful colleague to bring people along with them, Sonya does have that. And the other thing that the chief resident needs is to be the star quarterback, and that is what Amy has. So it’s a wonderful combination of the character qualities that those two people have built into those roles, and they complement each other well.”
Jake and Amy’s Fight Exposes Deeper Problems
While Joan handled leadership changes, Amy and Jake’s relationship took a serious hit. The couple treated a pregnant teenager together, and their disagreement over the patient’s care quickly turned personal.
Jake called rank on Amy during the case, fighting her in front of the patient instead of stepping outside to discuss their differences. Amy brought up Jake keeping secrets from her, which was out of line. Jake responded by bringing up Amy’s relationship with her daughter, Katie (Charlotte Fountain-Jardim).
The argument revealed deeper issues. Amy’s trust in Jake has been damaged, and Jake feels like he makes all the allowances in their relationship. Fans on discussion forums noted that both had valid points but handled the situation poorly.

Joan also confronted Richard about his behavior, agreeing with him that she has strong feelings about him being back at Westside. Huffman described Joan’s view of Richard in stark terms.
“I mean, that guy, even in every piece of dialogue they give him, that guy is just out for himself. I mean, even his great acts of, ‘I saved this person’s life,’ are all about serving himself. So he can’t be trusted.”
Huffman also shared that Joan wished she had more time to finish what she started at Westside. “The roof isn’t quite on the house she wanted to build. I think ideally she would’ve liked to have stayed another year until Amy’s really on her feet and Amy can take over, or Amy can go on and be the superstar somewhere else that she can be, but she wants to shepherd Amy through this transition.”
