The final chapter of the One Chicago crossover arrived on March 4, 2026, and it brought a huge secret to light. Hank Voight had to face a painful truth from his past involving the deadly Heart of Chicago fire. The episode also marked the return of Jesse Lee Soffer as Jay Halstead and Tracy Spiridakos as Hailey Upton, making the night a major event for fans .
The Crisis That Brought Everyone Back
The episode, titled “Reckoning (III),” picked up right where Chicago Med left off. A nerve agent was on the loose, putting the entire city at risk. The Intelligence Unit, now including Officer Eva Imani as Voights partner, worked with the FBI to track down the man responsible, a suspect named Thomas .
As the team dug deeper, they discovered that the attack was personal. Thomas was not just a random criminal. He was a survivor of the Heart of Chicago fire, a tragic blaze that killed 22 people years ago. Thomas suffered severe burns in that fire and lost his family. His grief and anger turned him into a monster, and he was now seeking revenge on the first responders he blamed for the system’s failures .
Voights Long-Buried Secret Revealed
The investigation took a sharp turn when the team uncovered Voights connection to the fire. Years ago, Voight knew there was a meth lab operating in the building where the fire started. He had been trying to catch a major drug dealer and worked with fire officials to delay a fire inspection for a few days. The fire broke out before the inspection could happen .
This secret had been buried for years, but the current case forced it out into the open. Officer Kevin Atwater was the one who found the evidence, and he hesitated to share it with the team. Halstead, who just returned to Chicago, immediately assumed the worst about Voight. However, Hailey Upton quickly shut down Halstead’s assumptions, reminding him that he had not been around for a long time .
When Voight learned that the truth could help them find an antidote and save lives, he did not hesitate. He told his team to use the information, even if it meant he would go to prison for his role in the cover-up. Following his orders, Upton and Imani confirmed the story to a reporter, exposing Voights involvement to the public.

Voight Faces the Survivor
The most powerful moment of the night came when Voight finally came face-to-face with Thomas. Instead of treating him like a criminal, Voight spoke to him as the scared little boy he once was. Voight validated his pain and admitted that everything Thomas said was true. He offered no excuses for what happened .
For a moment, it looked like Thomas might find peace. But the situation spiraled out of control. Halstead managed to get the nerve agent away from Thomas, but Thomas then pulled a gun on Upton. With no other choice, Voight shot and killed Thomas to save her life .
The weight of the moment was clear on Voights face. He had failed to save this boy once, and now he had to kill the man he became. After it was over, Voight placed Thomas’s file in a box with the other victims of the Heart of Chicago fire, a quiet sign that he saw Thomas as another casualty of that tragedy.

Halstead and Uptons Unfinished Business
While the case was the focus, the return of Halstead and Upton gave fans a look at where these characters are now. Halstead came back from Bolivia, where he had been working drug interdiction. Upton is now an FBI agent, and her investigation into Thomas led her back to Chicago .
Halstead tried to talk to Upton about their past during the middle of the crisis, but she was not interested. She made it clear that now was not the time. Later, Halstead apologized for leaving the way he did. “I’m sorry I lost you,” he told her. Upton admitted she was sorry too .
In the end, she offered him an olive branch by inviting him for a drink before they both had to leave. The two left the office together, leaving the door open for more conversations in the future. Tracy Spiridakos told Us Weekly that Upton was deeply hurt by how things ended, and while the door is open, it is not necessarily a new beginning.

After the crisis was over, Halstead spoke with Voight. He pointed out that the fire was not entirely Voights fault. The meth lab started the fire, and Voight was trying to stop a major drug dealer. But Voight still took the blame. He feels it is his sin to carry .
Halstead looked at him and said, “You’re different.” Voight has spent this season reckoning with his past and the people he has hurt. This episode proved that he is willing to face the consequences of his actions, even if it destroys his career .
