The Scrubs revival season finale aired on April 15, 2026, leaving fans with major questions about their favorite characters. The episode titled My Celebrations brought back old faces and set up big changes at Sacred Heart Hospital. The series brings back the original cast after nearly two decades, picking up with Zach Braff as JD Dorian, Donald Faison as Chris Turk, and Sarah Chalke as Elliot Reid now serving as the experienced doctors training a new generation of interns.
Dr Cox Health Diagnosis Creates Season Finale Cliffhanger
John C. McGinley returned as Dr Perry Cox, but his character faces a serious health battle. The penultimate episode revealed Cox has microscopic polyangiitis, an autoimmune disease that attacks the kidneys. JD wanted to treat his mentor right away, but Cox avoided getting tested to see if the treatment was working.
In the finale episode, Cox finally agreed to the tests. The results came back bad. The treatment was not working as hoped. Instead of sharing the news with his wife Jordan, played by Christa Miller, Cox wanted to keep it a secret. He told JD to hide the truth from her.
Jordan sensed something was wrong. She showed up at JD house to get answers. JD tried to protect Cox secret, but Jordan kept pushing. When she confronted Cox directly, he finally broke down and told her everything.
“I love the way you see me, I didn’t want to lose that,” Cox told Jordan.
Jordan replied, “If it helps, I’ve always seen you as a whiny baby man. Why don’t you let me be the strong one for once?”
The episode ended without showing what happens next to Cox. His health remains unclear as the credits rolled.
Christa Miller returned as Jordan for the finale. Showrunner Aseem Batra told The Hollywood Reporter that Miller wants to do more episodes if the show continues. McGinley also said he hopes to return for another season.
Janitor Makes Surprise Comeback in New Role
Neil Flynn returned as the Janitor in the Scrubs revival finale, but with a major change. He no longer works as a janitor at Sacred Heart. Instead, he now serves as the chief custodian for the joint commission, coming to the hospital for safety inspections.
JD initially felt confident when he saw the Janitor. He said, “I’m the chief of medicine now, you have no power over me.” But the Janitor had plenty of power in his new job. He pointed out safety problems throughout the hospital, including calling a toaster “basically a bomb” in a place with compressed oxygen.
The biggest twist came when JD met the new maintenance guy at Sacred Heart. Darcy Michael plays the Janitor son. JD got along well with him until he joked about the son being a “nepo janitor.” Both father and son took offense to the comment.
“You’re gonna pay for this,” Maintenance Guy told JD.
“Order restored,” the Janitor said cheerfully.

JD Finds New Romance with Rachel Bilson Character Charlie
Rachel Bilson joined the Scrubs revival cast as Charlie, a new love interest for JD. They first met at the hospital gift shop where Charlie complimented JD on his “shockingly smooth hands.” JD awkwardly credited his moisturizing gloves.
Turk and Carla tried to help JD by being his wingman, but their efforts backfired. They made it look like they were trying to pick up Charlie themselves. JD later realized that Charlie grandmother was one of his patients, giving them another chance to connect.
JD invited Charlie to a patient wedding reception at the hospital. At the party, Charlie asked blunt questions about JD past with Elliot. JD felt uncomfortable but stayed interested. When JD had to leave to check on Dr Cox, Charlie kissed him.
“Oh, you think I’m going to be turned on because you have to go save a life? Well, you’re right,” Charlie said before kissing JD.
Batra told The Hollywood Reporter that Bilson and Braff have worked together before on the 2006 movie The Last Kiss. She said they have natural chemistry that will show more if the show continues. Batra also mentioned that a second season would explore JD dating at this point in his life, dealing with baggage from his divorce and having a kid.
Sacred Heart Hosts Wedding After Elliot Saves the Day
The medical case in the finale focused on an older man who passed out during his daughter wedding rehearsal dinner. Elliot Reid, played by Sarah Chalke, wanted to fix him up so he could attend the wedding the next day.
The interns felt burned out and lacked motivation. JD tried giving them an inspiring speech about difficult roads leading to beautiful destinations. It did not work. Elliot stepped up instead and rallied the team. They fixed the man bowel blockage, and he seemed healthy enough for the wedding.
But the man condition worsened again. Doctors found a heart problem that needed a pacemaker. He would miss his daughter wedding. Turk suggested holding the wedding right at Sacred Heart Hospital. Dr Park, played by Joel Kim Booster, officiated the ceremony because he got ordained through an online church.
The wedding turned out sweet and gave the interns a much needed energy boost. The newlywed couple invited the whole Sacred Heart team to their reception.
Fan Response to Dr Cox Cliffhanger
Viewers expressed worry about Dr Cox fate on social media after the finale. Many fans took to X (formerly Twitter) to share their concerns.
One fan wrote, “Wait Dr. Cox isn’t going to be regular, or Carla? Oh, no.”
Another posted, “If Dr. Cox comes back to Scrubs as a patient I will riot!!!”

On Reddit, users asked whether Cox would retire or get a new job on the show. Many demanded answers about his future.
Bill Lawrence, the original series creator, spoke to Deadline earlier about Cox role. He said, “Dr. Cox is a huge character on the show. He comes back at the end of the year and will continue on next year.”
The finale ended with JD getting a text from Cox saying, “I could really use a friend.” JD felt moved to help the man who finally admitted he needed support. But then another text came saying, “Sorry, that wasn’t for you.” JD replied, “Too late,” before leaving the reception.
ABC has not yet announced if the Scrubs revival will return for a second season. Ratings for the premiere grew to over 10 million cross-platform viewers across four weeks, which gives reason for optimism about the show future.
