The new Korean drama Doctor on the Edge arrived on June 1, 2026, bringing viewers a mix of medical emergencies, personal trauma, and unexpected humor. The show follows a plastic surgeon forced to work on a remote island, where nothing goes according to plan from the very first moment he steps on the ferry.
The premiere episode, which runs for 70 minutes, introduces Do Ji-ui (Lee Jae-wook) as a public health doctor completing his mandatory military service in medically underserved areas. His three-year term takes him to Pyeongdong Island, a place no one wants to go.
A Military Training Start That Reveals Ji-ui’s Skills
The episode opens at a military training base where two soldiers get injured during wall climbing practice. One soldier dislocates his shoulder while the other suffers deep cuts. Do Ji-ui rushes to help, introducing himself as a public health doctor. He carefully stitches the wounded soldier’s cuts, showing steady hands and sharp medical knowledge.
The injured soldier thanks him, mentioning that his view of public health doctors has changed. Other soldiers talk about where they want to be placed. Ji-ui says he will go anywhere except an island. One soldier warns him about three things to watch for on islands: incidents, people, and love. Ji-ui brushes off the warning, believing he will not get sent to an island.
The Ferry Hallucination That Changes Everything
At the ferry terminal, Ji-ui appears reluctant to board. He takes out medication and swallows two pills. A doctor friend calls, warning him not to take the medicine due to dangerous side effects. The friend also mentions something about “Hwayeong,” though the show does not explain this reference yet.
A woman walks past while talking loudly on her phone, saying scars heal with money and demanding cash from the person on the other end. Her conversation oddly mirrors what Ji-ui just discussed with his friend. She boards the ferry, and a hesitant Ji-ui follows.
On the ferry, Ji-ui struggles to rest as three older women disturb him. He stares blankly, clearly not fully present. Then he sees the woman from earlier jump overboard. Without hesitation, he dives in to save her. But after hitting the water, he realizes nothing happened. The entire incident was a hallucination.
He wakes up at the island’s public health center. Staff members find him there and later explain he collapsed on the ferry. A young woman carried him to safety on her shoulders, though Ji-ui has no memory of this.
Meeting the Island Health Center Team
Ji-ui gets a tour of the health center. He first meets Dr. Yong Jucheon from the Korean Medicine ward, who seems overly excited about the new arrival. Jucheon immediately asks for Ji-ui’s phone number to add him to multiple group chats, hoping to help him adjust to island life.
The head nurse shows Ji-ui his office, Exam Room Number 1, which happens to be the largest room. But the equipment is all outdated. The nurse explains they request new supplies every year but receive nothing. Most patients have chronic illnesses and simply live with their conditions rather than seeking proper treatment.
Ji-ui requests a new blood pressure monitor. He then heads to his room above the health center. As he opens the door, a centipede falls directly onto his face. He runs out screaming. The nurses offer bug spray, and he sprays his entire room until it becomes uninhabitable.
The Switched Suitcase and Ha-ri’s Introduction
Ji-ui unpacks but quickly realizes the luggage does not belong to him. He goes to the village hall and asks the village chief about the switched suitcase. The chief mentions it likely got mixed up with Grandmother Mija’s granddaughter’s bag. That granddaughter is the same woman who carried Ji-ui off the ferry when he collapsed.
Ji-ui sets out to find her with vague directions: “a house with the red rooftop.” While searching, the woman appears again, screaming for help as a dog chases her. Ji-ui runs away terrified. The woman follows, and the dog starts licking her instead of attacking. A middle-aged woman arrives to retrieve the dog.
The woman introduces herself as Yook Ha-ri (Shin Ye-eun) . She and Ji-ui exchange suitcases. When she opens her bag to check the contents, the centipede from earlier crawls out. Unlike Ji-ui, who panicked, Ha-ri calmly picks it up and puts it aside.
Ha-ri hands Ji-ui his medication bottle and warns him not to take the pills because they do not suit him. He brings up the ferry incident where he saw her jump. She realizes he hallucinated everything and explains what really happened: her scarf flew off, and when Ji-ui tried to jump into the water to save it, she knocked him hard on the head to stop him.
Ji-ui refuses to believe her version of events. He takes his bag and leaves, unwilling to accept that he hallucinated so vividly.
Meeting Hyeon Chiyeon and Awkward Dinner
Back at the health center, Ji-ui walks in on a naked man in the bathroom. The man, Hyeon Chiyeon, shows no embarrassment despite Ji-ui clearly being uncomfortable. Chiyeon introduces himself and also mentions the ferry incident, which Ji-ui still does not remember.
Jucheon drops by Ji-ui’s room and insists the staff go out for dinner to celebrate his first day. The three men head to the island’s only restaurant. The owner is the same woman with the dog from earlier.
During dinner, Ji-ui reveals he is a plastic surgeon. Chiyeon bluntly says he has no interest in becoming a plastic surgeon because he sees them as money grabbers. Ji-ui sits stunned by the direct insult.
The food arrives, and Ji-ui looks visibly uncomfortable. Jucheon warns him this is the only restaurant on the island, so he will starve if he does not pretend to enjoy it. The owner waits for his review, and Ji-ui fakes enthusiasm.
A Life-Saving Emergency That Changes Everything
The restaurant gets a call from the health center. A patient needs a doctor, and Ji-ui volunteers to return. He finds Mr. Park, the village chief, sitting on the steps outside. Mr. Park complains of indigestion and requests a specific medication.
Ji-ui insists on doing an examination inside. He quickly discovers Mr. Park is not suffering from indigestion at all. The man is having a myocardial infarction – a heart attack. Ji-ui gives him nitroglycerin (NTG), which shows effects within minutes and confirms his suspicion.
Ji-ui tells Mr. Park he needs to go to a larger hospital immediately. Mr. Park refuses to listen. Ji-ui explains he could die without proper care. The village chief finally calms down but then sneaks away while Ji-ui makes an emergency call.
Ji-ui finds Mr. Park by his boat, trying to lift a heavy box. The man collapses. Ji-ui performs CPR until a helicopter arrives to transport Mr. Park to Asong Hospital on the mainland. Ji-ui rides along to ensure the patient receives proper care.
Stranded Overnight and Panic Attack at the Dock
After successfully getting Mr. Park admitted to the emergency ward, Ji-ui realizes the last ferry back to the island has already left. The head nurse tells him to find a motel for the night. He gets a room and some ramen but falls asleep without eating.
The next morning, his phone explodes with calls from the health center. Patients have gathered, and the staff threatens to report him for desertion if he does not return immediately. One warning for abandoning his post will bar him from transferring provinces, meaning he would stay stuck on the island for his entire three-year service period.
Ji-ui rushes back to the ferry terminal. But as he prepares to board, flashbacks of traumatic events hit him. He freezes, unable to move forward. Ha-ri appears and puts headphones on him, helping him calm down. The episode ends with Ji-ui looking surprised as Ha-ri stands beside him at the terminal.
What the Episode Reveals About Ji-ui’s Trauma
The premiere episode makes it clear Do Ji-ui carries deep emotional wounds. His hallucinations on the ferry, his panic at boarding the vessel, and his flashbacks all point to a tragic past involving water or islands. He previously mentioned the name “Hwayeong,” likely connected to someone he lost or failed to save.
The show also hints that Ji-ui may have been involved in a past medical failure. His friend warned him not to take medication that he clearly relies on. And his extreme reaction to simply stepping onto a ferry suggests trauma severe enough to affect his daily functioning.
Fans online have already started discussing Lee Jae-wook’s performance. Some viewers noted his character transformation feels like a perfect fit, while others admitted his visual chemistry with Shin Ye-eun still seems unfamiliar based on early photos. The production team has promised their relationship will develop further as they treat patients together at the health center.
